11. Certificate as grantee of ESC, if applicable (for Grade 11)

A.2 Transferees

3. Certificate of Good Moral Character (for transferees only)

1. Permit to cross enroll from the school where the student is currently enrolled

 Foreign Students

1. Five copies of the Student’s Personal History Form statement duly signed by the student, accompanied by his/her personal seal, his left and right thumb-prints and a 2”x2” ID photograph with plain white background taken not more than six months prior to submission
2. Transcript of Records/Scholastic Records duly authenticated by the Philippine Foreign Service Post (PFSP) located in the student-applicant’s country of origin or legal residence.
3. Notarized Affidavit of Support including bank statements (or notarized grant for institutional scholars), to cover expenses for the student’s accommodation and subsistence, as well as school dues and other incidental expenses
4. Police clearance certificate issued by the National Police Authorities in the student’s country of origin duly authenticated by the PFSP
5. Quarantine Medical examination by the National Quarantine Office
6. Photocopy of student’s passport
7. Birth certificate
8. Appropriate visa

  Married Students (female) additional requirement:
